Previous Address for eVetting Process

Hope I can get some insights about the eVetting process here. Thank you for your help in advance.

1. Do i need to provide all previous addresses I have lived in my entire life in the eVetting?

If so,
2. One of the places where I lived for 6 years when I was a kid has been completely demolished, now it's a school. How should I write this address?

3. When I was student in uni, I stayed in a school dormitory, do I also need to provide the address of the school dormitory?
