Application form help

Dear Guru,

I am going to apply Tier2 visa for me and my spouse next week. Please can you help me to answer following questions from application form?

Questions in Tier2 Application Form:

1)
Previous passport or travel document
Country of issue and issuing authority
I am Indian national and I renewed my passport at Indian High commission London. So what is the right answer to above question?

2)
I have 5.5 bands in all modal of IELTS. It is equivalent to B2 CEF level and it is sufficient for Tier2 general application. ?

Questions in Dependent Application Form:

1)
C4; What is the reference number for the most recent application of the PBS main applicant you are dependant on? (ht)
We are applying same time so I do not have reference number of most recent application of the PBS main applicant.

What type of supporting documents i have to provide for spouse application ?

I'm about to apply for tier 2 general visa with a complete set of documentation related to me. What all related to my previous employment do I need to produce if I have to, as I have quite a few of them (letter for job offer, appointment letter, confirmation, promotions, pay slips, form 16s etc).. lot of them as I've been here for many years. How many months payslips do I need to produce (3, 6 or 12 months)?

I don't see that the UKBA ever asked for my employment records but I have a lot of them, just need some advise on what to produce. Or do I go in with the full bunch?

Wrt my spouse's application, do I just submit a marriage certificate (in english), her passport which doesn't contain my name as her spouse because it was issued prior to our marriage and then her bank statement along with biometric, photos and TB test report?

She's currently employed with an MNC and does she need to be relieved from the job to be able to apply along with me as a dependant? Also, do we need to submit her educational certificates etc. I just want to be sure and do the documentation part right.

For a T2G entry clearance application your previous employment record in India is not relevant

yes (all originals)

She's currently employed with an MNC and does she need to be relieved from the job to be able to apply along with me as a dependant?
she does not have to give up her job just for the application but will of course need to comply with the terms of her present employment contract as regards giving notice, etc.
Also, do we need to submit her educational certificates etc.
