Tier 2 child born in uk applying visa from india

Hi everyone .
iam on Tier 2 (general) ,we are blessed with a baby here, travelling to india and apply visa from there.
iam confused with online form .
i have selected 1.work
2.tier2 general child up to 3 years.
please some one clarify me wethere i selected right form .if i selected right form then there are few questions which are are very confusing
1.What is your permanent residential address and contact details How long have you lived at this address shall i put india address if then he neverstayed there his first visit to india since he born (11Months)
2. Have you ever travelled to the UK in the last 10 years?
he is born here .so what shall i write.
3.Spouse / PartnerWhat is your marital status?
he is infant do i select single from the menu.
4.Have you ever received medical treatment in the UK?
he is born here in hospital but dont know what write about treatment .
5 .Dependent ChildrenDo you have any dependent children? Do you have any dependent children?
Yes* No

please clarify me wether i selected the right form .
