PBS Dependant of Tier 2 General

Hi,

Both my wife and I are on Tier 2 General. We have recently had a son and my wife is going to quit her job and move to being my dependant. I would like her to apply in person (using the same day service). We have both been on Tier 2 for over 2 years. I have a few questions

1. How much maintenance funds do I have to show and for how long do they have to be in the account?
2. Other than join bank account statements and marriage certificate is there anything else that I need to give as documentary evidence?
3. On the PBS dependant form I cannot find a place to tick £1001 which is the same day service fees? I remember there used to be both fees listed the last time I applied. Do I simply book an appointment online and show up with this form marked with £601 but they take £1001 or how does it work? I would be great if someone could outline the steps of the same day service process and the forms I need to fill in.
