Dependent Visa for Child born in UK

Dear Mods. Kindly do help me with these queres.

Situation: I am currently resident in UK with a Tier-2(general) visa and my daughter was born in UK this year.
I am in the process of applying for her Dependent visa from India.

Question. In the Application form how do we answer these questions -

1. What is your permanent residential address and contact details ?
Since, I am filing this dependant visa from India, am I supposed to give Indian address or UK address, for my baby ?

2. Have you been issued any visa for the UK in the last 10 years ?
My baby was born in the UK but does not have any visa.

3.Have you ever travelled to the UK in the last 10 years ?

5.Have you ever received medical treatment in the UK ?
