IHS refund swicthing from Tier 2 main to dependent

Can you please share the link to apply for IHS fees refund application.

Applicant and depenedents has approved 3 year visa.

If the Dependent is applying as a main visa holder, how to request for IHS fee on the previous visa for unused period.
