tier 2 general extension: required documents

I'm on a Tier 2 with my wife on Tier 2 dependant, my employer extended my contract and has issue a new CoS. We have a baby girl who is born in the UK and currently she does not have any visa.

What are the required document that I need to send for my application, my wife's application and my baby's application?
