PBS Dependent extension( Main applicant is ILR)

Hi ,

I am going to apply for PBS dependent extension (My visa expires on 05/10/2018) next month while my wife got ILR last year via Tier 2 General Route. I have couple of questions while filling the online form

1. There's a question regarding COS - What is the length of Main applicants COS. I believe my wife Cos was 3 years and 2 months but it is not valid any more since she got ILR. What should i select? Can select as 3 years or less?
2. How much of IHS charge i shouldto pay. since my wife ILR expirs on Oct 2024?

Sorry for asking the question.

I'm in the same situation and applying for the PBS dependent visa for my partner as I got ILR recently. I'm unable to find the correct PBS dependent visa application form for my partner.

Could you please point me to the correct PBS dependent form, please?
