Confusion in Part 6 PBS Tier 4

6.4 says Have you been granted a visa in last 10 years?????
like i got the visa in 2003 and then 3 extensions for leave to remain / shall i counted them as a visa but in 6.8 they asking for extension so just would like to confirm that visa and leave to remain is different?
