When filling out the application, it says to lists all countries visited in the past 10 years. Do I need to list all of them? I cant remember all the places I have gone.

I have my current passport, but not my old passport. Will that be a problem? I can list all the countries visited from the current passport, but don't remember from the old one as I had lost it.
