Timeline - First Entry to Citizen

Hi,

I'm really confused here:

To apply to be a citizen (naturalisation by residence), you must have lived here for 5 years.

Is it to say you must you have worked here for 5 years? i.e. you must have had a visa already? If so, aren't things like good character etc assessed at the point of visa eligibility? How about when one takes the knowledge in the UK test? Is it after getting PR? Can one apply to be naturalised when you are not a pr?

Enter Britain -> Work for 5 years, Apply for PR/ Indefinite Leave -> Wait 1 year 1 day -> Apply to be Naturalised?

I am most probably confusing myself here...ty
