FORM NQ help

Hi

I am requesting UKBA to provide a letter for not having British citizenship for my child by using the form NQ (Below link)

As I am applying for my child ( 5 years) old , can father / mother sign this form on behalf of child?. This letter is required by Indian high commission to renew the Indian passport for my child.

Any help is appreciated.
