Different addresses form AN

Hi All
I am in process of filling AN form based on 3
year spouse route (seperated)

I am staying with friends couch surfing for 2 years, until l sort myself out.
I have stayed at 2 different addresses how would l state this in 5 year address section of the application
Any advice would be appreciated.
