Is referee form used as is?

Hi Everyone,

I have a question regarding the referee form (below link). Is it to be used as it is including the first paragraph that mentions some requirements? Or I should copy and paste the referee declaration part only into a new blank page and put the photo?

Print it, glue the photo on the corner and take it to your referee for signature. When the referees has filled his section and signed, scan it and upload it with the rest of your documents. Keep the original in a safe place but don't send it anywhere.
