Previous applications on the form

Hello

(1) I need your valuable advise and help in filling up the previous visa applications section.

Between FEB- NOV 2016 I varied applications total four times.

I applied FLRO in 2016 for which I enrolled biometric.
After two months my application was varied to FLRFP and after its two months it was again varied to FLRO for both I never enrolled biometric. Finally in 2016 I again varied to PBS dependent and I got the visa.

Do I have to mention all visa applications in my form?

(2) I got 3 points on my license for speeding four years ago. It was not by court. I think now points are gone and not showing on my license. Do I have to mention this on the penalty section?
