Advice needed: Referee did not sign the declaration form

I'm looking for your advice if you have relevant experience!

I did get "sure, I'll sign the form!" from my referee... and I did submit my application form (don't do like me).
A few weeks forward and now I do not have the signed form! Not sure if too busy or don't want to sign it after all which is ok, but would be good to have an answer. Anyway.

I see two options here:
1. cancel my application and complete another one with a different referee there. Not sure if the fee paid would be refunded + will lose time and appointment slot for biometrics.
2. get another referee and have a signed form from them - add a note to the completed application, explaining the situation with the referee who has disappeared for an unknown reason)

Please advise!
