"Previous application details" questions

We are asked about "Previous application details" on the online application. I'm unsure about the following:

1. One of my previous applications was a Tier 1 (Exceptional Talent) visa. At least at the time I applied (Spring 2018), this was broken into a Stage 1 application to which only the primary applicant (me) applied, and a Stage 2 application including myself and a dependent applicant. It looks like the Stage 1 and Stage 2 applications have different unique application numbers. Should I list both the Stage 1 and 2 applications separately for myself (and presumably my dependent should list Stage 2 only)? Or is only stage 2 the "immigration application"?

2. If the answer to the above is "list both separately" - I followed all the directions given me the time and gave biometrics when required, but I do not recall whether biometrics were required separately for stages 1 and 2 - would fingerprints have been taken as part of each stage? (The question "Were your fingerprints taken as part of this application?" is asked of each application).

3. Our first application was made outside of the UK - having paid & submitted an online Tier 2 application on 14 Sep 2014, we then had an initial appointment at a USCIS COLO Application Support Center in Milwaukee on 23 Sep; we were then told to mail the application and supporting documents to the British Consulate in New York where they were received on 29 September 2014. Would "Where was it made?" be Milwaukee or New York? Would "Date of application, if known" be 14 Sep 2014 or 29 Sep 2014?

That is where and when you applied.
