MN1 reference expiry

Hi All,
I will probably be applying my four year old son’s certificate (MN1) in October. He has an appointment with the GP in couple of weeks and I wanted to find out if I can get the form signed now? Is there any guidance that how old the signature can be? Also I want to apply via NCS so that I can keep hold of my passport.
