1.17 - I would work my way backwards but it doesn't really matter. Put the best dates you can.
1.21 - leave blank if you are happy to attend a ceremony near your home address.
1.22 - tick first option (settlement).

GP is fine. The second referee doesn't need to be professional if over 25. Just British. If they don't want to put their passport number, they can put "available upon request"

Referees should either be:
1) a person of any nationality who is of professional standing such as a minister of religion, civil servant or a member of a professional association such as an accountant or a solicitor; or
2) A holder of a British citizen passport and either a professional person or aged 25 years or over.
Both your referees should know you personally for at least 3 years and are not related to you or to each other. They must not have any unspent convictions as well.
Here's the list of acceptable professional persons who can act as referees:

Both your manager and your friend qualify as a second referee, them both having British passports (and assuming that both of them also meet all other requirements to become a referee). However, I can't give you a definite answer if both or any of them can qualify as your first referee. You have been given the list. Just please try to find out if their qualifications fit in the requirement.
