AN Form Query

A quick question,
On the naturalisation certificate there are two numbers,
1. Certificate Number:XXXXXXXX
2. Reference Number: XXXXXXXX (Home Office Reference Number)
On form AN section 1.34 where it asks
If your partner is a Naturalised British citizen, give citizenship certificate reference number.
Which of the two numbers should be put on the AN form?
With regards to addresses for past 5 years, should we put the current address again even though applicant has lived here for over 5 years?
From - will be from the date moved in 10 years ago
To - still living here - so what date should be entered here?
