MN1 application

Would I have to fill up and submit two separate MN1 forms for my two kids?
Section 1:
1.2 -- leaving it blank
1.3 -- leaving it blank
1.5 -- Is that Surname only? They did not ask for first name anywhere on the form (my daugther name is Zara Ahmed) Zara firstname and Ahmed Surname
1.21 - 1.39 -- Thats my and my wife's details (to be filled up only which r relevant)
Section 2
2.1 - leaving blank...?
2.2 - Both my kids r under 5.....do I still need to fill up section 2?
Section 3 -- Skipping
Section 4 -- Skipping
Section 5 -- Do I need two referees for each child? GP and ?
Please advise I need to complete and submit this ASAP
