Hi all, i am using form MN1 to register my two kids born in UK under section 1(3). One kid is 3 yrs old another just 9 weeks. We have been with a same surgery for more than 3.5 years however the GP and Health visitor both have declined to sign as 1st referee because they dont want to give there Passport numbers?

What are my chances for application to be accepted WITHOUT passport number being mentioned or text like ( available upon request ) written instead ??

It is understandable that they don't want to provide their passport number and "available upon request" is acceptable by the HO. You should have no issues with it.
