Filling MN1 form - queries

All of us (my husband, daughter and myself ) have ILR in sep 2011 and now applying for naturalisation.
MN1 Form queries
in section 1.2 filled in child's ILR number, is that correct?
section 1.6 filled in main applicant's telephone number and email address?
Residence Requirement
2.1 blank
2.2 present address as child is living here since birth(she is born in UK)
2.3 absent once from UK, have mentioned the details.
2.4 filled

WE DID NOT GET ANY CARD WHILE GRANTED ILR JUST STAMP ON PASSPORT. IS IT OK?

I will get her form signed by her health visitor and one of my colleague who I know him for more than 4 years. He is British. Please suggest if this set of referees will work.
we are going to get our application form signed by him too.

Please correct if I am wrong at any point and answer my queries.

section 1.6 filled in main applicant's telephone number and email address? OK.
