Naturalisation form

Hi there I am applying for the British Naturalisation. I am feeling the form out and I am a bit confused on the question below
2.1 Date and place of first arrival in the United Kingdom?
I am applying under the 3 years rule.
I came to the UK as a student January 2008 and went back to my country 14 of July 2008.
I came back as a student 2009 January again and extend my student visa 2009 October till September 2010. Then I got married and applied for a probation visa September 2010 and was valid till September 2012. Then I applied for my IRL August 2012 and got it. Now I am applying for the Naturalisation and I don't know what date should I put on question 2.1 in the application form dose my qualified period start from 2008 or 2009 ? I am a bit confused Has anybody had same situation ? Also on the application form it asks for the past 5 years addresses it it for the 3 year rule as well do I have to put the last 5 years or 3 years of addresses on the form ?
