Child registration - MN1 form

All,
I need to register my child as BC, he was born in Aug 2012.
I have read several posts but still not clear on few things, so I would like to ask them again.
Do I need to complete 1.2 (Immigration and Nationality Directorate or Border and Immigration Agency or UKBA reference number)?
I have currently mentioned the number mentioned on my BRP card
What do I need to as child's 'present nationality' in 1.9?
This is the most confusing. Shall I mention as Indian which he is not but eligible for due to both parents being.
What referees shall I use?
Both my GP and health visitor has refused. So, I have used a doctor (not GP). Second reference is also professional (Property Consultant). Both are British and over the age of 25. Both have mentioned that they know the child through their parents. I hope this is going to be OK.
What documents do I need to submit along with the application?
I am aware of child's birth registration certificate, parent's marriage certificate and ILR proof.
Is it possible to use NCS?
As I travel internationally for work, I cannot afford to send my BRP for up to six months if it is required.
