I am sorry if this is already answered, I searched but couldnt find a definitive answer.

I am a limited company director on minimum salary and take dividends.

Do I need to provide the self-assessment statement of account?

There are contradicting posts on here so I am not really sure what the answer is.

Also do I need to provide the UTR number or the PAYE reference number?

The UTR should suffice though there is nothing stopping you putting in the statement of accounts, many directors are also self employed.

You can absolutely apply as an employee of your LTD company as you draw salary from the company. Just provide PAYE ref. no.
