Documents to Submit

I was granted ILR on 4 Jan 2012 on the basis of spending 5 years on Work Permit and Tier 1 General combined. And I have spent 1 year on ILR now.
What documents do applicants usually submit for the application?I could not find a list as such, if there is a list could provide me with a link? From what I read, it was
1)Passport
2)Life in the UK Test Pass Certificate
I would really appreciate it if some one provides me with an answer

fill the form pay the fee of £850 1 photo your passport and oriinal life uk .

Fee is £874.
You might need to send accountant letter if you are self employed.
Also if you have used any other id than your passport when you took your LIUK test then you need to send that as well.
