***how to fill IN tables in 2.4 and 2.5 from application?

hello, I am an EEA national so I need to fill in 2 tables in 2.4 and 2.5
but its a bit confusing
in question 2.4 they ask for: Please provide details of employment, selfemployment, school or college:, does it mean I have to write only: employment and nothing else in the table 2.5 I have to provide job title and adddress of employer?
One more thing regarding table 2.4 they ask: Please state on what basis you were in the United Kingdom for the last six years
in the table you have to write date from and date too, so if I write
dates to cover 6 years exactly, but in the middle of that period there will be a 2 week gap as I was on holiday and didnt have new address till I got back, is it OK or they will rejected as 2 weeks missing?

07/09/2007 - 10/09/2008 lONDON, WAITER
(NOW HERE GAP IN DATES)

ACTUALLY DO i HAVE TO WRITE 07/09/2013 OR JUST WRITE STILL IN TAHT EMPLOYMENT
