Hi Guys,

can someone please clarify

1- I thought there is ONE AN application form but I read here people taking about AN6 (1) or AN6 (2). I am applying on the basis of 3 yrs spouse of BC which form do I need to use?


2- On AN form question 1.31 : my wife had 3 different FAMILY names before, 1 when she was born then her first marriage family name then after devorce she changed her family name again. do I need to mention these 3 family names in the space provided? Just the family name of the full name?
