I'm about to apply for BC, I need help plz

Hi all, I want to thanks in advance for reading my post.
Finally it's over and time to apply for BC, I need some help please.
First, 1.2 is asking for passport/Travel doc number. I only have my national passport ( from Iraq ) and 1 expired travel document which I never even used. Do you have to write my national passport no or just leave it blank?
Second, I have recently changed my name ( Deep poll ) my driving licence, LIUK certificate new name but my Iraqi passport still old name ( don't need to change it and waste money ) and my language ESOL certificate old name. My question is, on the AN application can I use my name name? I will include a copy of my deep poll ( change of name ) certificate.
Third, 1.3 Please say when you received your indefinite leave to remain! I have Permanent Resident! is that the same?
