Britsh Citizenship after ILR

HI All,

I am currently on ILR which was granted to me Feb 2013.
History
Came to UK on HSMP in Feb 2008 (2 years)
1st Extension on Feb 2010 (3 years)
ILR on Feb 2013

Can some one Advice me what is the process going forward for citizenship.

I have IELTS which i did in 2008 for entry clearance and my understanding is that its enough even after the expiry date of 2 years.
Also me and my Wife have LIUK passed test certificates.

What form we need to use Me and my Wife along with 2 kids.

Many Thanks.

I have gone through the details and few more questions

I left my Permanent job around June/July last year after getting my ILR in feb

now i am working as a director of my own company and my wife is also director.

The form says in case of director or self employed we need to give self assessment Tax details which we submitted to HRMC.

but we have not done so far
what options do i have now , we are taking tax free salary from the company and dividends but we havn't done our tax returns yet
should i wait for the returns to be submitted or can i show my salary slips(tax free) and dividends vouchers
