Form AN and supporting documents

Hi all,
Hope you are well. I've got a few questions.
1. In the supporting documents section of the form, you're told to tick the documents submitted. For the evidence of identity, i used my BRP for Life in the UK test. However, it is not listed as one of the options. Do you just write 'Residence Permit' for example down or do i have to tick what's there?
2. I have got a UK degree to satisfy the Knowledge of English. Is my Degree Certificate the only thing that's needed as evidence? That's all i've got.
3. I first entered the uk in 2003 on a Student visa. Since then, i've had a number of visas without leaving the country for long. But I have put 2003 as first date of entry. Is that correct? (That intention to live long term mentioned in the guide is a bit confusing)
