What FORM??

Hello, I'm looking for some help! (Aren't we all!)

Right, my time line:
I am American. Been in UK working/living for 6 and a half years.
Married Irish (southern) guy in 2006 August.
Although he's Irish with an Irish passport, he was born in Oxford, UK so has a British passport as well.
We moved to UK in Oct 2007 , me on a a Spouse Visa (in hindsight, bad move, the spouse visa, not the relocation)
Lived together in UK until Divorce July 2009
We have a child together who was born 1 year before we married. She now has a British passport (only needed her father's birth certificate to get it).
My Spouse Visa expired August 2009, but had applied for EEA2 Permanent residence card as family member of EEA national just prior to SV expiration.
Perm residence card/sticker in passport issued Jan 2010 and good until Dec 31st 2014
I had professional advice that I could apply for something (can't remember if its naturalisation, PR or ILR) once I had been here 5 years ON THE PR card though, not a TOTAL of 5 years (e.g. my first 2 years here 'didn't count towards residency' because I was on a Spouse visa).
So, apparently I can apply for something (end goal is naturalisation/British passport) but question is What and When? And what forms?
If I must wait to apply for PR/Naturalisation until my 5 years on PR card (Jan 2015), then it will have expired. Does that matter?
If I can apply now for PR or better yet, naturalisation, then I'd like to take my KoLL test and pay my fees (£900?) and get it done asap. So What form do I need? EEA3? EEA4? or AN?

FYI - I meet all the requirements of sound mind, working, contributing, English speaking, not lived outside UK for x amount of days etc.

In addition, do I need to do anything else about my daughter's status? I assume since she has a British passport, she's considered a citizen already!
