AN form question-civil judgements under Immigration Act

Hi
Re. Point 3.8 AN application- civil judgements under immigration act
My visa was once refused with right to appeal as I had not maintained required bank balance when applying. I appealed and the appeal was granted and subsequently I was granted the visa. All this happened within lawful residence. I then got my ILR in 2012 under ten year long residence. My question is:
1) does this refusal count as a civil judgement?
2) do I need to mention this in the form under 3.8 ?

Also I did my life in the UK test in 2011 when applying for ILR - would this be still valid?
And I have completed a MBA degree in the UK - would this qualify for English language requirement?

Would be grateful for any advice.
