Photograph - Referees and Identity section of the AN form

Hi,
It may be a stupid question though just want to clarify. Have the photograph should be pasted in the Section
5:Referees and Identity section of the AN form ? It has came to mind if the photo needs to be pasted in the space given then why they want the name and DOB written in back of the photograph as they would not know if its written in the back of the photograph or not
