AN Form - Quick Question

hello,
in form AN - I am not self employed and my employer pays my tax via PAYE. Do I have have to answer question 3.1 to 3.5?
If yes, where can I find my tax reference number and the tax office address (Question 3.5)

Ask your company accountant.

My first question was: Do I have have to answer question 3.1 to 3.5?

Yes you do.
You can find company's tax reference on your P60 or ask your company's payroll dept.
