Sec 4, the spaces are not enough to put my father' name, how can i deal with it? Thanks for your help.

Do i need to send Birth certificate as well? or just previous passport is enough.

I've become British Citizen by Naturalization after ILR, Now in order to apply for passport do i need to submit my Birth Certificate. If i can't get it what should i do.
