Naturalisation after PR

thanks a lot. Do i need to send my p60 with AN form (AN form and Guide AN don't mention that) or only if they request it during the process?

I passed CEFR level b1.1
on certificate it says:
Grade 5 Granded Examination in Spoken English
Entry Level Certificate in ESOL International (Speaking and Listening) (Entry 3)
CEFR Level B1.1
then three number on the bottom:
Qualification number:500/193.....
Trinity ID: 1-485709.....
Candidate number: 1-4908....
On AN form which should i put as:
1. "title of qualification"
2."level of qualification"
3."award reference number"
Sorry if I'm being thick just don't want to fail the process because of stupid mistake
I wonder why the cannot make it more clear on the certificate and use the same terms as on the AN form
