Self employment history for Spouse

Hi All,
Thank you ever so much for all help we get from this forum.
my wife is applying for nationalization as spouse of British citizen. She has been here for 4 years and 8 months. She only started working after 1 year arriving here. So what should i put for that year and also she was self employed for 8 months for which we paid the tax and eveything. What should we put for those dates.
I would appreciate if you can put the informatoin in the following format.
From To Occupation Employeer Name Employer Address
