An Form questions

Hi seniors,

I am filling An form and I don't know the answers of two quesions .
1. Date and place of your first arrival in the UK?
I came to this country in July/2004 as a student and left in Oct/2008 after completing my Master degree. Again in August2009 I came back on post study work visa and in December/2009 I switched to Tier 1 General.
Now I am cofuse what date should I write in this section? The guidence notes says write the date when you came for long stay.

2. Second question is what is your tax office adress or refence?. I have recently started self employment and prior to this I was full time employee and also a director and employee of my own limited company. I do have my UTR and also do Self Assessment every year.
I don't know what to write in this section.

Please help.

They ask the date of entry with a view of staying in the UK permanently so that will be the date from December 2009

I don't know what to write in this section.
When employed you should take it from your P60. When self employed ask your accountant

Thanks for your reply. I don't have have accountant. I do all HMRC work by my self.
No worries I will from my accountant friend.
