form mn1 referee

my daughter 12.5 years for registration filling up form mn1 section 3(1). one referee has to health visitor or teacher. school doesnt have any policy of filling up the form. gp says shr hasnt been into the surgery for a long time. as she is 12 been for vaccination a long time in another city. so who will up the referee section anyone from professional list solicitor accountant british national etc or has to be her teacher or nurse on her gp or dentist. teacher has refused and itsso difficult to get appointment with dentist n gp n not sure if they will agree to do that.

If none of the usual suspects will agree then yes, ask anyone you know from the acceptable list.
