Advice needed: Dual National changing name in one passport

Hi,

I am dual national (British/Iranian). I have recently legally changed my name by deed poll and would like to change it in my passport. Unfortunately I can't change my name in my Iranian passport, as expected. I can not renounce my citizenship (impossibly difficult).

I discussed the issue with HMPO and they have advised I send all of the usual documents + deed poll, statutory declaration and a signed affidavit detailing my history (citizenship), documents being sent and why I can't change my name on my Iranian passport.

I don't have a statuary declaration regarding my name change. Firstly can I prepare a form based on this template and have it signed by Justice of the Peace (JPS) (scotland):

Furthermore I have changed my name a few months ago by deed poll, what date do I put in this?

Would JPS be able to sign my affidavit as well.
