Register my child born abroad as citizen or direct passport?

Hi, so this all seems somewhat confusing to me.
I am a british citizen not by descent as I understand it (born in the UK to british parents/grandparents etc) I have been travelling and working abroad for a good 3-4 years now and am currently in Chile where I have a 2 year old son with a Chilean Passport, we are travelling to the UK for a couple of months and I want to make sure I take advantage of that time to apply for his British passport.
My question is do I need to register/apply for his citizenship first with the MN1 form and then apply for his passport?
Or is he a "british citizen by descent" automatically and I just need to apply for his passport?
Likewise what is the situation with him regarding the NHS? Does he need to complete the above processes before he is entitled to healthcare or is it automatic?
Although we will be travelling with insurance it's still something I'd like to know going in just encase anything happens and I have to make any decisions I'd like to be as informed as possible as to not waste time.
