Naturalisation as a family member of an EEA national

Hi, I currently hold only a European passport, which I got at the start of this year.
Before the European passport, I had a Indian passport which contains my permanent residence card.
I got my permanent residence card, as a result of my dads european passport and him exercising his treaty right.
I had to surrender my Indian passport as India do not allow for dual citizenship. Is it ok to have my permanent residence in my old passport or should I transfer it to my European passport.
I have been living in the UK for over 15 years and now I would like to apply for naturalisation.
For the section about exercising treaty right should I show my proof e.g Uni/ college letters or use my dads p60 since I got the permanent residence based on him exercising his treaty right.
About comprehensive sickness insurance do I need that?
I have called up NCS and they are asking for my proof of exercising treaty right. Is this correct? I also called up home office, and they have suggested to use my dads p60. I am very confused. What should I do?
