Minor questions on form binding, english req. & cover letter

Hi all.

I'm scheduled to submit my application using NCS next week and I had some last minute "minor" questions.

1. How would it be best to bind the application form? Just staple it or maybe use a two-hole binder or folder?

2. Would it be a good idea to include a cover letter that states what is included (e.g. English certificate, P60s and HMRC letters, etc.) as well as clarifying aspects of the application such as the fact that my first referee is acting as a director of a VAT registered company?

3. Finally, what should I tick for question 1.22 (english requirement) as I have both the English B1 certification and I met the requirement English requirement for settlement before 28/10/2013 (using that certificate)? Should I just tick the latter but still include the certificate? Or maybe tick both?

1) Staple; (keep simple)

Keep copy of form and payment form, NCS won't copy those for you.

2) Contents list can't hurt.
Keep any explanations simple and straightforward.
Explaining referee is probably not necessary.

3) Is the B1 a valid SELT?

On the third question, yes, it's the Trinity College one that I took last year for my ILR application.
