Naturalisation for elderly mother

Hi All,
Need your advise and help here. My mom came here on elderly dependent visa and completed her 5 years stay included ILR .now I am going to apply her Naturalisation and British passport. Could you please advise on the following query.
1 - Due to age and health she is unable to attempt life in the UK and language exam. So what kind of evidence I need to submit to waive these requirements?
2- What is the name of the form for her naturlisation/ citizenship?
3- Any further information and documents required for her application?

How old is your mother?

Thanks Casa for your reply.
She is 62 years old but she is constant on medication and unable to attempt these exams. I can get doctor letters on medical conditions.
Also I want to mention here , she was living here with me on visitor visa from past several years. She got her elderly dependent visa in March 2014 as indefinite live to remain here. So do you think she is eligible to apply her citizenship and passport now? Do you think her stay here as a visitor will count in 5 years route?

How long has she stayed in the UK on visitors visa (give as much detail as possible)? Did she leave the country for atleast 180 days a years before reentering the country? Did she at any time breach the terms of the visit visa?

Maybe you should list the 'past several years on visitor visa' detail before anyone can offer advice and how she managed to live here for several years on a visitor visa.
If she got ILR in 2014, she won't qualify for BC till 2019.
She will only be exempt from LIUK and English test when she is 65. To be honest, 62 years old is not really that old.

Dear All,
I checked her passport and correct information are as below:
- She got her indefinite leave to remain visa (based on elderly dependent visa) on 3 Oct 2011 and arrived in the UK in 17 Feb 2012
- She has been outside in total 436 days from UK so far.
So what would be the safest date to apply for her naturalization?
Also in the following link, its clearly mentioned that if the applicant has health issue then life in the UK and English exams can be exempted.

She is from Pakistan , on medication from NHS from long time and have several serious health issues. I would like to know which kind of documents or evidence I can provide to prove this criteria?

No earlier than 17th February 2017 onwards.

Your definition of 'serious health issues' and HO's definition of 'serious health issues' will likely differ greatly.

Dear All,
Thanks for your replies and these are really helpful. However, I am still trying to get clear understanding for my mom's life in the UK and English test.
Following sites says people who came under Elderly Dependent Visa category (under paragraph 317 of the Immigration Rules )are exempted from these tests. I mentioned clearly in my first post that she came in this country on Elderly dependent visa, as she is widow, had sever health conditions and long medical issues.

You cannot provide the link for the IMMIGRATION RULES when asking about citizenship which is a different set of LAWS! They are different.
