Potentially silly quesiton. So we are asked to say if we have ever been known by other names.

I've used my real name in all official documentation: at University, at work etc.

However in some cases (EBay, Amazon etc) I'm using the English version of my first name. For example Mikel -> Michael. Card details are obviously using my real name, but all Amazon, Tesco, Ebay etc. letters have the English version of my name.

Do I need to declare this in the AN form?

No you don't. They mean official other names (like a woman who got married or someone who legally changed their name).
