Help on AN Form Filling 6-Biometric Enrolment

6- Biometric Enrolment:
as I am using NCS service and plan to keep my BPR. what is the answer for questions bellow:
6.5 place of issue? the card was issued in Liverpool UK- would it be the correct answer.( this address was in the letter they sent me with the card attached to it).
6.7 BPR enclosed? answer (NO) and if no, state the location.
Question - Can I just fill my address as the location of the card.
then there are these options:
returned go to 6.8
lost 6.9
stolen 6.10
other 6.12
I reckon I should tick 6.12, is this correct?
and 6.12 says: if BPR is not enclosed please give details why you are unable to provide it.
not sure what to answer as this would not be the case, but would keep it in case I need to travel.
