section 8 of passport application query

I am filling the passport application along with the form AN. My first name is long and is more than 30 characters. The passport guidance mentioned that "If you do not have enough space to write
your name or contact details (for example,you have a long surname or a long email address), please use section 8 of the application form". So I have to mention my full name and mobile number in Section 8.
I am having the below queries
1) Do I need to write the text "NAME" followed by full name (first name+surname) in first two lines of section 8 or just write only the full name without any text ?
2) Do I need to specify any text like "MOBILE" prior to writing mobile number in last line ?
