The company I work for requested me to be self employed and do my own self assessment. Where do I get these info from.

An employee? A business partner? Self-employed? A director?
Answer: I believe should be Self-employed.Correct?

1.49 Address of tax office or tax office reference
Answer:

1.50 Employment history in UK during past 10 years, or since date of entry if you have been
here for less than 10 years (continue on page 22 and use additional sheets if needed).
From To Occupation Employer Name Employer address.
I assume this is not applicable to me as Self Employed. What would be your answer?

6. Biometric Enrolment

As a EU Citizen I was issued with a Blue Card and it says on it:
Document Certifying Permanent Residence.
I am not sure if them both as the same thing.

The question on the AN for is:

6.1 Have you been issued with a BRP with a previous application for leave?
Answer:

Yes go to question 6.2 No go to question 6.16

Answer:
For this I have requested a SA302 from HMRC as self employed but employment history would take up to 40 days to receive it from HMRC.
